What's it called?

Give me your first choice and two backups. Names get rejected more often than people expect, alternates mean we file same-day instead of starting over by email.

How the check works: I run every name against the New York State entity database and confirm with you before filing, within one business day, usually much faster. No online checker is final; the state decides at filing, and similar-sounding names can block yours even without an exact match.

The county matters more than people realize, for LLCs it sets the newspaper publication cost New York requires.

Name and mailing address for each member or shareholder.

No Social Security numbers here. The EIN application needs the responsible party's SSN or ITIN. I collect that by phone only, never through a web form. If any site asks you to type an SSN into a signup form, leave that site.
Each owner needs a name and mailing address.

    We confirm the entity choice, I collect the one piece of information the EIN application needs (that's why it's a call), and you approve the quote.

    Documents go to the state the same day. EIN usually lands within the hour. You get everything by email as it happens.

BEFORE LAUNCH: (1) Supabase keys live in site.config.json, and the build fills them into this page, portal.html and admin.html. Submissions land in your `leads` table and appear in the New leads panel of the admin console. Until the keys are set, the form shows the visitor a message to copy and email instead. (2) Verify the NY state filing fees in STATE_FEES against the current DOS fee schedule. (3) Verify county publication ranges in PUB_ESTIMATES. Queens is researched ($700-1,450); the others are directional and must be confirmed by calling county clerks before you quote them. (4) Replace hello@kodexandco.com via personalize.ps1.
